Housing Stabilization Services, a Medical Assistance offering, aims to support individuals with disabilities, seniors, and those facing homelessness risks in locating and maintaining housing. These services are integrated into the state plan for Home and Community-Based Services (HCBS), rather than being a waiver. HSS comprises three distinct services: Housing Consultation, Housing Transition, and Housing Sustaining.
This service focuses on crafting a person-centered plan for individuals who do not receive MA case management services. It aids individuals in constructing a housing- focused, person-centered plan and facilitates informed decisions regarding housing transitions or sustaining providers. Additionally, consultation services help individuals connect with resources outlined in their personalized plans that are not related to housing.
